We are only accepting new patients aged 6-25 for initiation/management of medication
Notice:
- Our practice only accepts patients 6-25 years of age.
- Our practice cannot complete short-term or long-term disability for patients who have not been seen for at least 6 months.
- Our providers DO NOT prescribe or manage benzodiazapines (clonazepam [Klonopin], diazepam [Valium], lorazepam [Ativan], etc.), buprenorphine-naloxone (Suboxone), clozapine (Clozaril), ketamine, Spravato, or stimulants (amphetamine-dextroamphetamine [Adderall], dexmethylphenidate [Focalin], methylphenidate [Concerta, Ritalin], lisdexamfetamine [Vyvanse], etc.).
NEW PATIENT REGISTRATION STEPS
- Check insurance plans we accept and potential fees
- Complete Patient Portal Registration.
- Complete the Consent form.
- Complete the Intake form.
- Submit your photo ID, insurance card(s), and any other documents.
- Pay the $50 deposit and schedule an initial psychiatric evaluation.

Required Documents:
- Photo ID
- Front AND back of insurance card(s)
Optional Documents:
- Release of Information
- Medication List
- Medical Records (including lab/EKG results)
- Legal Documents

Please contact us to pay your $50 deposit and schedule an initial appointment.
